We love sharing our patients’ smile transformations. This form gives Beyond Braces permission to use photos or videos of you or your child on our website, social media and other materials. Signing is optional and does not affect your care in any way.
Before You Start
- Patients 18 or older can sign for themselves.
- For patients under 18, a parent or guardian signs.
