Consent for X-Rays, 3D Scans and Photos
We use digital 3D imaging (Cone Beam CT) and iTero intraoral scans to see your teeth, roots and jaws in detail. These images help Dr. Dhol make an accurate diagnosis and plan your treatment. This form explains the imaging and progress photos we take and asks for your consent.
Before You Start
- Radiation exposure is kept low with a focused x-ray beam, sensitive sensors and a protective apron.
- Progress photos are taken to track and compare your treatment.
- For patients under 18, a parent or guardian signs.
